What Is Behavioral Health Case Management?
Behavioral health case management is about more than just connecting people to services—it’s about walking alongside them as they navigate the challenges of recovery and daily life. A case manager works collaboratively with individuals to assess their needs, create a plan of care, and coordinate with providers who can best support their well-being. This may include help with medical and dental care, legal matters, benefits, transportation, and community or support group connections.
At Experience Structured Living, a certified behavioral health case manager in San Diego will work closely with you to understand your unique needs.
We provide compassionate, non-judgmental guidance to help you access the resources you need and feel supported every step of the way.
Community vs. Private Case Management Services
Case management services vary significantly between models. Community case management, provided by government agencies or nonprofits, focuses on basic services like assessment and referral while private case management services offer more comprehensive support, including crisis intervention, advocacy, counseling coordination, and specialized referrals. Smaller caseloads that enable intensive, personalized care are just one of the benefits of private case management.
At Experience Structured Living, we offer private case management with two distinct levels:
- Baseline housing support that provides foundational behavioral health support services like daily routine assistance, medication management, and appointment coordination.
- Enhanced private case management that goes further with specialized one-on-one work focused on building independence, developing recovery skills, and advancing occupational goals for long-term success.
How ESL's Case Management Program Works
Case management begins with a comprehensive assessment of your needs. Your mental health case manager then creates a customized plan, coordinates services with providers, and provides ongoing advocacy, education, and referrals to help you achieve your goals.
Our private case management services include three core components:
- Independent Living Skills Development: We offer financial literacy, budgeting, meal planning, cooking, public transportation navigation, and time management that build confidence in everyday tasks.
- Recovery Support Services: We encourage recovery group engagement, sponsor connections, relapse prevention planning, mood tracking, and accountability systems that support sustained recovery.
- Occupational Development: We help with resume development, interview coaching, career planning, volunteer placement, certification programs, and academic exploration to help individuals obtain meaningful employment.
Who Benefits Most from Private Case Management?
Case management for mental health at Experienced Structured Living is especially beneficial for individuals who’ve tried multiple treatments without lasting results, those struggling with independence and needing more than basic coordination, and families seeking centralized communication and peace of mind.
Our program helps you transition from structured treatment to independent living with consistent accountability, family support, and a focus on relapse prevention, life skills, and vocational progress.
What to Expect from our Private Mental Health Case Managers

They can help individuals understand the services they need and how to access them, as well as provide advice on managing symptoms and improving overall well-being.
