What is Case Management?
Case management is a collaborative process of assessing, planning, facilitating, and advocating for services that meet an individual’s needs. It supports individuals in gaining access to resources and helps them achieve their goals. Through case management, case managers help individuals identify the services they need and coordinate those services with providers that are best suited for meeting their unique needs. A case manager in San Diego will also work closely with the individual to offer support for legal issues, medical and dental concerns, acquiring benefits, transportation needs, and self-help support group strategy so they are receiving the best possible care.
The behavioral health case management team at Experience Structured Living is committed to providing personalized case management services. Our case managers provide compassionate, non-judgmental support as they assist individuals in navigating the behavioral health system.
Community Vs. Private Case Management Services
When it comes to case management services, there is a big difference between community case management and private case management. Community case management typically focuses on providing basic case management services such as assessment and referral. Private case management offers more comprehensive services that can include crisis intervention, advocacy, counseling, education, information, and referrals. Private case managers are also often better equipped to provide personalized case management services that address the individual’s unique needs and goals.
How does Case Management Work
Case management begins with an assessment of the individual and their needs. Based on this assessment, case managers create a plan that outlines the services they need and how those services will be coordinated with providers. The case manager then provides advocacy to ensure that the individual receives all necessary services and resources to meet their goals. Additionally, case managers provide education, information, and referrals to ensure that the individual is receiving the best possible care.

What to Expect from a Mental Health Case Manager
When seeking services from a mental health case manager, it is important to know what to expect. Case managers will assess the individual’s mental health needs and create an individualized plan based on the findings of the assessment. This plan may include referrals to services such as counseling, therapy, support groups, or other resources that can help the individual achieve their desired outcomes. Case managers are also often available to provide advocacy and education regarding mental health issues. They can help individuals understand the services they need and how to access them, as well as provide advice on managing symptoms and improving overall well-being.
